Invokana, Invokamet, and Invokamet XR Linked to Gangrene
Invokana, Invokamet, and Invokamet XR are sodium-glucose cotransporter-2 (SGLT2) inhibitors, a class of medications used to treat Type 2 diabetes. SGLT2 inhibitors help to lower a patient’s blood sugar by preventing the kidneys from absorbing glucose and helping them eliminate sugars through the urine.
On September 29, 2018, the U.S. Food and Drug Administration (FDA) issued a Drug Safety Communication explaining that SGLT2 inhibitors such as Invokana, Invokamet, and Invokamet XR have been linked to cases of necrotizing fasciitis of the perineum, also known as Fournier’s Gangrene, a serious infection of the genitals and their surrounding areas. In response, the FDA has required new warnings be added to prescription information and the patient Medication Guide.
The rare but life-threatening bacterial infection affects the muscles, nerves, tissue, and blood vessels in the genital and anal areas. Symptoms of Fournier’s Gangrene include:
- Swelling, redness, or tenderness of the genitals
- Fever of 100.4 degrees Fahrenheit or higher
- General malaise

In June 2016, the FDA issued a Drug Safety Communication regarding the increased risk of kidney injury in patients taking Invokana and Invokamet, among other SGLT2 inhibitors. Acute kidney injury occurs when the kidneys stop working properly and can lead to an increased buildup of waste within the body.
